緩和ケアの国際的見直しと総合診療医の新たな役割

　世界保健機関（WHO）は、National Cancer Control Programmes^1）にて、緩和ケアを含む総合的癌対策のあり方について提言を行った。一方、私自身が所属する国際ホスピス緩和ケア学会（IAHPC）でもその緩和ケアマニュアルを改訂しながら、既存の緩和ケアの制約を乗り越える提言が繰り返しなされてきた^2）。本来の領域が血液腫瘍内科で精神科の研修を受けた経験もあることから、私自身オンコロジストおよびサイコオンコロジストの双方から包括的緩和ケアの実践・理論化を進め^{3, 4）}、WHOおよびIAHPCの提言に沿って、癌早期発見を含む癌サポーティブケア、実存的苦痛（existential distress）に対する心のケア、非癌患者への緩和ケアなど、従来の緩和ケアの概念を超える全人的なケア（holistic care）を提唱してきた^5〜7）。日本の緩和ケア^8）も改善に取り組んでいるが、困難が大きいだけに歩みが遅い。その問題解決を加速させるため、2018年にWHOとIAHPCによる共同プロジェクトとして、緩和ケアの定義見直しが開始された^9）。私はIAHPCの2014年度Recognizing Loyalty Awardを受賞したことから、このWHO-IAHPC共同プロジェクトの情報を共有することとなった。
　本稿では、その新たな緩和ケアの見直しの国際的潮流の中から3点、つまり、地域コミュニティにおける総合診療医への期待、既存の癌治療による緩和治療、および癌治療と同時の緩和ケアについて、簡単に述べる。
