病歴
患者：54歳、女性
主訴：発熱、下痢
現病歴：入院10日前から4日間、県外で単身赴任中の夫宅で過ごした。6日前より38℃台の発熱、1日5回程度の水様性下痢、両側大腿外側の紅斑が出現（図1）。紅斑は1日中続き、消退する時間はなかった。4日前、咽頭痛と嚥下時痛が出現。3日前、発熱が続き、下痢が30回程度に増えたため、近医を受診。急性腸炎の疑いで抗菌薬（セフジトレン）とロキソプロフェン、整腸薬を処方された。その後も症状が改善しないため、当院を受診した。
ROS陰性：悪寒戦慄、頭痛、鼻水、咳嗽、喀痰、呼吸困難、嘔吐、腹痛、関節痛
既往歴：カニ鍋で蕁麻疹（15年前）
常用薬：なし
生活歴：喫煙なし、機会飲酒。海外渡航歴なし、ペット飼育歴なし、sick contactなし、生もの摂取なし。性交渉：夫のみ。最終1カ月前
