　悪性腫瘍の終末期と比べ、下降期心不全に対する薬物療法のエビデンスは少ない。慢性心不全に対して基本となる薬剤を可能な限り継続しつつ、終末期に向けて出現してくる呼吸困難、倦怠感、疼痛など各症状への緩和的対症療法が必要となる。本稿では、外来あるいは在宅で可能な治療として、主に内服薬を取り上げ、点滴投与する薬剤についても一部紹介する。
　なお、心不全は主にHFrEFやHFpEF（HFmrEFは省略）に分類されるが、根本的にHFpEFに対する効果的な治療は確立されておらず、薬剤のエビデンスも少ないため、リスク因子への介入が重要とされていることを先に述べておく。
