CASE
患者：78歳、男性。
主訴：発熱、右肩関節痛。
現病歴：結石性胆管炎にて消化器内科入院中。内視鏡的逆行性胆管ドレナージ・結石除去・抗菌薬加療を行い、入院時の心窩部痛と血液検査の肝胆道系上昇は改善がみられるものの、発熱・CRP上昇継続、1カ月前からの右肩関節痛も続いており、消化器内科研修中の初期研修医を通じて内科コンサルト。
併存症：結石性胆管炎（入院13日目）、低アルブミン血症。
既往歴：高血圧、高尿酸血症。
薬剤歴：点滴静注；セフメタゾール1g/12時間、内服；ロサルタン50mg 1錠、1日1回、ドキサゾシン1mg 1錠、1日2回。
喫煙・飲酒歴：なし。
職業：無職。
アレルギー歴：なし。
