❶急性呼吸窮迫症候群（ARDS）

　ARDS（acute respiratory distress syndrome）は、1960年代に認識されて以来、医学が大きく進歩してきたにもかかわらず依然として死亡率の高い症候群である。重症ARDSにおける死亡率は40％にものぼる^1）。
　そもそもARDSという疾患概念は、「何らかの傷害による肺の病的反応」とやや曖昧なものであり、臨床現場でもARDSという言葉を「急性呼吸不全を起こすさまざまな鑑別疾患のなかの診断名の1つ」なのか、「急性呼吸不全を起こしているさまざまな病態のなかの1つ」なのかを意識せず用いているのではないだろうか。概念の共通化を目的として作成された国際的な定義^2）は、「ARDSとは何か？」に答えてくれるものではなく、むしろ実臨床で鑑別を進めていく弊害の1つになっている感すらある。
