病歴
患者：82歳、男性
主訴：左上下肢麻痺
現病歴：来院15時間前までは普段と変わりなかった。来院4.5時間前の起床時にマットから起き上がれなかった。左口角から食べ物がこぼれ、歩行時に左下肢に力が入らず、左側に傾くことにも気づき受診した。
ROS（review of systems）陰性：頭痛、嘔吐、複視、視野障害、痙攣、発熱、意識障害、感覚障害、胸背部痛、頸部痛
既往歴：高血圧症、脂質異常症
薬剤歴：アムロジン5mg、ロスバスタチン2.5mg、アスコルビン酸3g、ランソプラゾール15mg、ベポタスチンベシル酸塩20mg、モサプリドク塩酸塩5mg、イルソグラジンマレイン酸4mg
喫煙歴：20〜50歳×20本/日
飲酒歴：機会飲酒
社会歴：ADL自立、妻と息子と3人暮らし
