㊵ 食後に様子がおかしい

患者：80歳、女性。
尿路感染で入院中。経過は安定しているが、食後に様子がおかしくなることに気づいた。症状は見当識障害から傾眠までさまざまだが、数時間で改善する。意識障害と判断して血糖値を複数回測定してみたが、190〜230mg/dL程度であった。食後の血圧低下は認めず、脈拍数も正常範囲だった。一般血液検査および血液ガスには異常を指摘できない。反復する可逆性意識障害として代謝性脳症を疑い精査を進めたところ、高アンモニア血症を認めた。肝疾患の既往はない。
