好酸球増多に“降参”しないための系統的アプローチ

　今回は宮古病院近隣の離島からの精査紹介のCASEであり（離島診療所では限られた検査資源のため、白血球数の分画評価も検体を当院に送り後日の検査報告となるため、評価・判断をするのにどうしても時間が必要となる）、患者は受診X-1月に当院入院歴がある。当時の入院時に好酸球増多症を認め、薬剤性を疑い薬剤中止後に改善した経緯がある。
　末梢血中の好酸球増多は、日常診療のなかで遭遇することが非常に多い。その原因疾患は、アレルギー性疾患、寄生虫感染症、ウイルス感染症、結核、自己免疫性疾患、骨髄増殖性疾患、リンパ増殖性疾患、副腎不全、薬剤性など多岐にわたる。また、好酸球は臓器浸潤性が高く、生命維持に必要な臓器に浸潤して臓器障害を引き起こすことがある。臓器障害の正確な評価と管理を行いつつ、系統的なアプローチを適切に行っていかなければ、生命予後や機能予後に影響を与える可能性がある。
