Case
患者：65歳、男性
主訴：目の上と顎の下が腫れてきた。唾液が出づらい。
現病歴：半年前から、食事の際の顎下部痛を自覚していた。歯科受診したが異常はないと言われ、経過観察していた。昨日、鏡を見て両顎の下と左上眼瞼が腫れていることに気づき（図1）、インターネットで調べ「IgG4関連疾患」という病名が出たので心配になり受診した。
　抗核抗体陰性、血清IgG 3,320mg/dL、血清IgG4 423mg/dLであり、顎下腺生検で線維化を伴う形質細胞浸潤を認め、免疫染色でIgG4/IgG比 65％であったことから、実際にIgG4関連疾患と診断した。
