Case1
「関節リウマチと思いきや？」その1
患者：70歳台、女性
主訴：手指の関節痛
現病歴：来院2カ月前から、朝に1時間程度持続する両手指のこわばり、右手首・右母指付け根の疼痛を自覚した。増悪傾向にあり、来院2週間前から右手関節の腫脹も出現したため受診した。
身体所見：右手関節に腫脹・圧痛あり。左第三指PIP（近位指節間）関節に圧痛あり。右側上腕二頭筋長頭に圧痛あり。下肢関節に異常所見なし。
画像所見：手指X線；明らかな骨びらんを認めない。関節エコー；右手関節の関節液貯留・左第三指PIP関節の滑膜肥厚（Ⅱ度）を認めた。
血液所見：WBC 12,000/μL（Neut 67％、Lym 16％、Mono 5.2％、Eos 2.8％、Baso 8.7％）、Hb 13.3g/dL、Plt 38×10⁴/μL、TP 7.0g/dL、Alb 4.2g/dL、AST 30IU/L、ALT 23IU/L、LDH 326IU/L、CPK 58IU/L、BUN 19mg/dL、Cr 0.66mg/dL、CRP 0.04mg/dL、抗CCP抗体 10.3U/mL
