Case
患者：70歳、男性。
喫煙歴：20本/日×50年。
現病歴：COPD（慢性閉塞性肺疾患）でかかりつけ医に通院中。ある冬の夕方に急に寒気があり、発熱と少し鼻汁が出るようになった。様子を見たが、38℃を超えたため夜間救急外来受診。インフルエンザ迅速抗原検査を行い、陰性であったため帰宅となった。翌日には起きるのもつらいような倦怠感があり、咳も出るようになり、再受診した。感冒にしては全身症状が強く、よく聴くと、同居している孫がインフルエンザに罹患していることが判明。接触歴、症状からインフルエンザの事前確率が高く、インフルエンザと診断した。全身症状が強く、年齢、基礎疾患のCOPDからインフルエンザ合併症発症のリスクが高いと判断したが、胸部X線で肺炎像はなく、抗インフルエンザ薬を処方した。
