Case（仮想）
患者：Aさん、58歳、男性
既往歴：人間ドックで左腎結石を指摘された。
現病歴：近未来のある平日の夜、左腰背部に激痛が突然出現した。痛みがおさまらず、翌朝かかりつけ医を受診した。腹部エコーで左腎盂に約1.6センチの結石が嵌頓しており、水腎症をきたしていると診断された。同日、泌尿器科を紹介受診したところ、外来担当医から「自然排石を期待して経過観察」「対外衝撃波腎尿管結石破砕術」「経尿道的尿路結石除去術」「経皮的腎砕石術」などの選択肢を提示された。
　同泌尿器科のAIは、医学的に最善の選択肢として「経尿道的尿路結石除去術」を推奨した。医師も丁寧な説明を追加した。しかしAさんは、精神的・身体的に冷静な比較考量ができる状況になかった。早く痛みをとってほしいばかりだった。
