病歴
患者：78歳、女性
主訴：右下腹部痛
現病歴：中等度の認知症をもつ78歳女性。夫によると、来院7日前から右肩痛を訴えていたが、鎮痛薬は使用せず消退した。来院1日前から腹部の違和感（本人は「調子が悪い」と表現）を訴えていた。来院当日、朝食の準備中より臍周囲の腹痛を訴え始めた。徐々に右下腹部の間欠痛となり、しゃがみこんで動けなくなったため救急搬送された。救急車中では茶色吐物の嘔吐が1回あった。悪寒・発熱、便通変化、血便・黒色便、腰背部痛、性器出血・帯下はない。
生もの・鶏肉・焼肉食はなかったが、来院2日前にしめ鯖を含む刺身を食べたという。周囲に腹痛や下痢を訴える者はいない。
既往歴：髄膜腫術後症候性痙攣、甲状腺機能低下症、骨粗鬆症
薬剤歴：カルバマゼピン200mg×2/日、レボチロキシン25μg/日、エルデカルシトール0.75μg/日、ミノドロン酸50mg/月
生活歴：京都出身で京都在住。旅行は長年しておらず、飲酒・喫煙はしない
