CASE 1
患者：56歳、女性。受診4カ月前、両手指中心に関節痛を自覚し、関節リウマチを疑われて紹介受診し、Disease Activity Score 28［DAS 28（ESR）］が5.34と、高疾患活動性の関節リウマチと診断される。
　B型ならびにC型肝炎ウイルスの既感染でないことを確認し、メトトレキサート（methotrexate：MTX、メトトレキセート^®）の内服開始。その後増量するも、依然として高疾患活動性であり、MTXに加え、インフリキシマブ（レミケード^®）を導入する方針となった。リンパ球数・IgG潜在性結核の有無を、病歴、胸部画像、QuantiFERON^®で評価した後、レミケード^®投与を開始した。その後、疾患活動性は低下して寛解に至り、以後8週間隔投与で低疾患活動性から寛解を維持している。
