脊椎・脊髄病変からの胸痛や腹痛

　圧迫骨折や椎間板ヘルニアでは胸痛や腹痛をきたすこともあるが、その場合、急性冠症候群や胆石発作などの内臓疾患としばしば誤診される^1）。原因不明の腹痛では前皮神経絞扼症候群（anterior ctaneous nerve entrapment syndrome：ACNES）が注目されているが、圧迫骨折や椎間板ヘルニアなどの脊椎病変が原因不明の腹痛の原因であることは、実はあまり知られていないかもしれない^2）。また、頸椎・頸髄病変からの胸痛はcervical anginaとして知られている^3）。
　本コラムでは、これらの脊椎・脊髄病変からの胸痛や腹痛の特徴について、症例を提示しながら解説する。
