アウトカムからみた心房細動のカテーテル・アブレーション

Point
・本邦における心房細動アブレーションの全国レジストリー（J-CARAF）によると，初回アブレーション後1年間の心房細動/粗動/頻拍の非再発率は発作性心房細動で70%程度，持続性/長期持続性心房細動で60%前後である．
・薬剤抵抗性の有症候性発作性心房細動患者に対するアブレーション治療は抗不整脈薬治療に比較しQOLを改善させ，その効果は長期にわたり持続する．
・心不全を合併する持続性心房細動に対する無作為化前向き試験（AATAC試験）で，心房細動アブレーションはアミオダロン治療と比較し非再発率が高く，予定外入院率および死亡率が低いことが示された．
・心房細動アブレーションを受けた患者では受けていない患者に比べて有意に死亡率・脳卒中発症率が低く，心房細動のない対照群の患者と発症率が同等であったとの報告もある．
