腎機能障害の高血圧．どこまでACE阻害薬・ARBは使えるのか？

Point
・ACE阻害薬・ARB投与による腎予後の改善は，輸出細動脈拡張により糸球体内圧を下げ，糸球体過剰濾過を改善することによると考えられている．
・ACE阻害薬・ARB投与開始初期（概ね3カ月以内）の30%以内のクレアチニンの上昇は，むしろその後の腎予後の改善を示しており，継続投与すべきである．また，同様の現象は，心保護作用にも報告されている．
・ACE阻害薬・ARB投与開始初期（概ね3カ月以内）の30%以上のクレアチニンの上昇は，腎動脈狭窄などを念頭に置き，いったん中止後精査が必要である．
