大動脈二尖弁に対するTAVI—これまでの臨床成績と適応の選択

Point
・大動脈二尖弁症例へのTAVIは,新世代デバイス(Sapien 3, Evolut R, Lotus valve)の使用により,三尖弁大動脈弁狭窄症に対するTAVIと同等の結果を得ることができる.
・大動脈二尖弁症例へのTAVIの成績は,主にレジストリーからの報告であり,適応には,ハートチームによる個々の症例に応じた適切な判断が必要である.
・大動脈二尖弁へのカテーテル人工弁とそのサイズ選択には,確立された方法はなく,supra-annulusの面積,周囲長,短径・長径,commissural lengthなど,aortic valve complex全体の解剖を考慮して決める必要がある.
