大動脈二尖弁に対するTAVI—これまでの臨床成績と適応の選択

Point
・大動脈二尖弁症例へのTAVIは，新世代デバイス（Sapien 3, Evolut R, Lotus valve）の使用により，三尖弁大動脈弁狭窄症に対するTAVIと同等の結果を得ることができる．
・大動脈二尖弁症例へのTAVIの成績は，主にレジストリーからの報告であり，適応には，ハートチームによる個々の症例に応じた適切な判断が必要である．
・大動脈二尖弁へのカテーテル人工弁とそのサイズ選択には，確立された方法はなく，supra-annulusの面積，周囲長，短径・長径，commissural lengthなど，aortic valve complex全体の解剖を考慮して決める必要がある．
