MitraClip—これまでのエビデンスと現在進行中のトライアル

Point
・MitraClipの適切な患者選択を検討するうえで，欧米における治療の現状とエビデンスを知ることは重要である．
・EVEREST Ⅱ試験は外科手術可能のMR例を対象とし，MitraClip群は外科治療に比し再手術が有意に多い結果であった．
・同試験は，高齢，機能性MR，EF60%未満の例では，外科治療とMitraClip治療の成績は同等であること，器質性MRの例では外科手術の成績が優れることを明らかにした．
・欧州の多施設レジストリでは高齢，機能性MR例を治療対象の主体とし，高い手技成功率とQOL改善が示されている．
・MitraClipはMR合併心不全の重要な治療選択肢として期待されるが，現時点では生命予後改善を示すデータはなく，今後の知見が待たれる．
