問診・理学所見でどう診る？

Point
・冠動脈疾患は専門医による問診のみで，かなり正しく診断できる．冠動脈疾患に特徴的な自覚症状だけでなく，非典型的な症状，大動脈解離や逆流性食道炎など鑑別疾患の典型的症状を知ることが重要である．
・冠動脈疾患の既往と家族歴，古典的冠危険因子の有無，重症度，治療歴の聴取により冠動脈疾患の検査前確率は向上する．
・冠動脈疾患に特異的な理学所見は少ない．鑑別疾患に特徴的な理学所見，全身の動脈硬化を示唆する身体所見に注意する．黄色腫や耳朶の皺（Frank徴候）は冠動脈疾患と関連する．
