収縮障害を示す疾患の鑑別

Point
・左室の局所壁運動異常は主に虚血性心疾患で認められ，冠動脈病変に伴ってその支配領域に出現する．
・たこつぼ型心筋症や心サルコイドーシスでは特徴的な局所壁運動異常が認められる．
・左室全体の壁運動異常が認められた際には，まず虚血性心疾患かどうか，そうでない場合には心筋炎によるものや二次性の心機能低下でないかを識別し，これらが除外されれば特発性拡張型心筋症と診断される．
