Point
・加齢に伴い心不全患者は増加するが，高齢心不全患者は，多臓器障害や認知症，サルコペニアやフレイルの合併といった医学的な問題だけでなく，独居や老老介護といった社会的問題も抱えている．
・高齢心不全患者に対する加療は難渋することが多いが，急性期病院から慢性期病院，かかりつけ医，在宅診療まで多職種によるチーム医療を継続的に行うことが重要である．
