QT延長症候群

Point
・先天性QT延長症候群（先天性LQTS）は，現在までに十数個の遺伝子型が報告されているが，1型，2型，3型LQTSが90%以上を占めている．β遮断薬は心イベントのリスクを低下させるエビデンスがあり，第1選択の薬物である．
・二次性LQTSは，本来のQT間隔は正常もしくは正常上限であるが，外的要因（薬剤，電解質異常，徐脈，器質的心疾患など）によってQT間隔の延長を認める場合に診断する．QT延長の原因を特定し，可及的速やかに除去および是正することが治療の基本である．
・無症状のQT延長を有する患者が初回イベントとして突然死もしくは心停止を発症することがあるので，初診時には適切に対応する必要がある．
