利尿薬を使い分ける

Point
・急性心不全に対する利尿薬投与の目的はfluid overloadを解消するためであり，フロセミドの静注を行う．
・慢性心不全に対する利尿薬投与の目的はEu volumeを維持するためであり，ループ利尿薬の経口投与を行う．
・ループ利尿薬で効果不十分な場合や，間質浮腫が著明な場合はトルバプタンを併用する．
・すべての利尿薬は漫然と投与せず，患者をとりまく環境変化に応じてその用量を調節する．
