急性心不全へどう静注薬を使いこなすか

Point
【静注血管拡張薬】
・血圧高値の心原性肺水腫症例に対しては，降圧作用の強いニトログリセリンを用いる．
・ニコランジルは降圧作用がマイルド．薬剤耐性は示さない．
・カルペリチドは血管拡張作用に加え，利尿作用，RAA系抑制および交感神経系抑制作用を併せもつ．
【静注強心薬】
・低心拍出，低灌流症例に対する第1選択薬としてドブタミンを選択する．
・低血圧を合併する際は，カテコラミン製剤にノルアドレナリンを併用する．
・高度低心拍出症例でドブタミン抵抗性の場合にPDE Ⅲ阻害薬を併用する．
