ジゴキシンを考察する

Point
・洞調律のHFrEF患者において，ジゴキシンは主に心不全入院を抑制する．第1選択薬であるACE阻害薬またはARBとβ遮断薬に必要に応じて追加してもよい．
・治療抵抗性の心房細動患者には，徐拍化のためにβ遮断薬を第1選択とするが，ジゴキシンの追加投与を検討してもよい．
・中毒を起こしやすい薬剤であり，血中濃度が高くならないようにモニタリングし，0.5〜0.8 ng/mlとする．
