新しい心不全治療薬①—イバブラジンが必要な場面

Point
・イバブラジンは洞結節のHCN4（I_f）チャネルに作用し，心拍数を低下させる．
・イバブラジンはSHIFT試験において，β遮断薬を含む心不全基本治療下の左室駆出率の低下した心不全（HFrEF）において心血管予後を改善させた．
・わが国では，心不全基本治療下のHFrEFにおいて，洞調律下に心拍数75 bpm以上の場合にイバブラジンの使用が可能である．
・イバブラジンは低用量から導入し，心拍数を見ながら増量することにより高度の徐脈の出現は避けることができる．
・イバブラジンによる心拍数低下は，心機能の改善・左室リバースリモデリングを伴う．
・血圧の低い症例でも血圧は保持されるため，比較的安心して使用できる．またβ遮断薬の開始・増量時にイバブラジンを併用すると，より安全かつ効果的にβ遮断薬を増量できる．
