静脈血栓塞栓症の抗凝固療法

ここが変わった！
●過去のスタンダード
　・未分画ヘパリンを調節投与して，ワルファリンの効果が安定するまで継続する．
　・ワルファリンは，PT-INRが1.5〜2.5となるように調節投与する．
●現在のスタンダード
　・安定例では，初期治療期，維持治療期に非経口抗凝固薬あるいはDOACを投与する．
　・全身状態，下肢症状，家庭環境，病院環境が適しているならば外来治療を推奨する．
