大動脈疾患—Stanford B型解離に対する治療方針

ここが変わった！
●過去のスタンダード
　・大動脈解離は発症から2週間以内を急性期，2週間以降を慢性期と定義されていた．
　・急性期の合併症を有する急性Stanford B型大動脈解離に対してのステントグラフト治療は推奨されていたが，合併症を有さないStanford B型急性大動脈解離は保存的な治療が行われていた．
●現在のスタンダード
　・2週間までを急性期，2週間から3カ月を亜急性期，3カ月以降を慢性期と定義された．
　・ステントグラフト治療の広がりとともに，合併症を有さない急性Stanford B型大動脈解離であっても将来の瘤拡大を予防する（preemptive）目的で，亜急性期から慢性早期でのpreemptive TEVAR（胸部ステントグラフト内挿術）が推奨されている．
