二次性（機能性）TRは手術的修復より薬物治療を優先すべきか？

POINT
・日本のガイドラインでは，特に心房細動による単独二次性TR症例では，右室機能低下が進行する前の早期介入が推奨される．
・薬物療法で粘りすぎると右室機能低下が進行し，周術期予後が悪化するため，右室機能低下の進行を早期に認知し手術介入したほうがよい．
・左心系疾患など薬物治療でTRの原因に介入可能な場合や少量の利尿薬が軽快する場合は，薬物治療を強化してフォローすることを推奨する．
・カテーテル治療が可能になれば，さらに早いタイミングで治療可能かもしれないが，適応は十分に考慮して（右心不全発症や逆流の重症度など）決断する必要がある．
