減塩は不要である

POINT
・心疾患患者には減塩食が必要だという一律な固定概念は振り払われるべき時期に来ている．
・高齢者への塩分制限食の継続は現実的に難しいことが多い．
・高齢心不全患者では，減塩指導より十分栄養をとるなどのフレイル/サルコペニアの予防のほうが重要であることが最近わかってきている．
・患者に指導する前に，われわれは日常の外来での心不全患者において，どの程度，患者の食塩摂取量を評価したうえで，栄養指導をできているのであろうか？
・高齢者ほど，通常の日常生活における塩分摂取状態を見極めた心不全薬物コントロールを考慮することが重要ではないか？
