●アドバンス・ディレクティブではなくアドバンス・ケア・プランニングを行う．
●Ask-Tell-Askアプローチを用いて患者の気持ちを引き出し，意思決定支援を行う．
●在宅で亡くなることが正しいわけではなく，状況に合わせた対話を行っていく．
