心不全地域連携におけるハートノートの活用

POINT
●心不全は管理が難しく，再入院率が高いとされている．
●加えて，循環器領域では大都市圏での広域・複数の中核病院が賛同する地域医療連携は運用されにくい．
●大阪では2017年より「大阪心不全地域医療連携の会」が活動しており，循環器領域での地域医療連携の新しい取り組みとなる．
●本稿では本会の取り組みを通じて心不全地域ネットワークの必要性について論じたい．
