POINT
●訪問リハビリは通院困難な高齢心不全患者の受け皿になり得る．
●訪問リハビリでは個々の身体・生活状況，家屋環境に応じた直接的な介入ができる．
●生活の場に直接伺う訪問リハビリにより，心不全の再入院予防が期待できる．
