心機能の経時的変化　リモデリングとリバースリモデリングとは？

POINT
・心筋障害，エネルギー代謝異常，神経体液性因子の影響で心拡大，心機能障害が進行するリモデリングが生じうる．
・リモデリングの原因となる因子が取り除かれることによって，リバースリモデリングが生じる例がある．
・心不全後に心拡大/心機能障害が改善したHFrecEFという病態が存在し，同様にLVEFが保たれたHFpEFとは区別すべきである．
・HFrecEFにおいても心不全治療薬は継続すべきである．
・リモデリング，リバースリモデリングの診断，経過観察にはストレインイメージングも含めた心エコー図によるフォローが重要である．
