左室流出路狭窄に対する治療　術前心エコー評価のポイントは？

POINT
・閉塞性肥大型心筋症のLVOTOの重要な成因は，中隔肥厚，SAM，異常筋束である．
・HCM患者の半数以上に心尖部〜中隔基部に付着する異常筋束を認める．
・侵襲的治療適応のあるHOCM患者で有意なMRを認めた際は，MRの成因評価のためTEEを考慮する．
・成人のLVOTOの原因疾患として，大動脈弁下狭窄，僧帽弁副組織があることを知る．
・LVOTOの診断はTTE，TEEで行う．
・LVOTOの成因評価にはTEEが有用である．
・構造異常の評価には心エコー検査の他，MRI，CTといったマルチモダリティでの評価が有用である．
