MR合併例に対するMitraClip®とカテーテルアブレーションの選択

POINT
●心房細動が長期に持続すると,心房・弁輪拡大を引き起こし,心房性機能性MR/TRを起こす.
●心房細動に対するカテーテルアブレーションにより洞調律が維持されると,心房性機能性MRが減少するという報告があり,心房性機能性MRの症例においてカテーテルアブレーションは「弁膜症治療のガイドライン2020年版」において,Class Ⅱaで推奨されている.
●重症心房性機能性MRへのカテーテルアブレーションは洞調律維持が困難である場合が多く,高齢者などのハイリスク症例ではMRへのカテーテル治療MitraClip®も含めてハートチームで検討することが重要である.
