MR合併例に対するMitraClip^®とカテーテルアブレーションの選択

POINT
●心房細動が長期に持続すると，心房・弁輪拡大を引き起こし，心房性機能性MR/TRを起こす．
●心房細動に対するカテーテルアブレーションにより洞調律が維持されると，心房性機能性MRが減少するという報告があり，心房性機能性MRの症例においてカテーテルアブレーションは「弁膜症治療のガイドライン2020年版」において，Class Ⅱaで推奨されている．
●重症心房性機能性MRへのカテーテルアブレーションは洞調律維持が困難である場合が多く，高齢者などのハイリスク症例ではMRへのカテーテル治療MitraClip^®も含めてハートチームで検討することが重要である．
