A case of relapsing polychondritis

Relapsing polychondritis （RP）is a rare, chronic inflammatory disease characterized by recurrent and progressive involvement of cartilage and proteoglycan-rich tissues throughout the body. Because the symptoms often occur in the head and neck region, otorhinolaryngology is often the first department to be consulted. The frequency of the disease is low and symptoms are varied, and there are no specific laboratory findings, making diagnosis and treatment difficult. In this report, we describe our experience of a case of RP in which treatment was initiated promptly in collaboration with rheumatologists.
はじめに
再発性多発軟骨炎（relapsing polychondritis：RP）は全身の軟骨やプロテオグリカンを多量に含む組織が再発性かつ進行的に侵され，多彩な症状を呈するまれな慢性炎症性疾患であり1），本邦での指定難病にも登録されている2）。初発症状としては耳介軟骨炎が最も多く，そのほか気道，眼球，関節などにも生じる3）。頭頸部領域に症状を生じることが多いため，耳鼻咽喉科に最初に受診されることも少なくない。その発生頻度は低く症状も多彩であり，特異的な検査所見も存在しないため診断に苦慮する例もみられる。また，治療も確立されたものはなく，膠原病内科を中心に経験的な治療が行われているのが現状である。今回われわれは膠原病内科と連携して迅速に治療を開始しえたRPの症例を経験したため報告する。
