３) 尿からの食事蛋白摂取量の推定と患者の栄養状態指標

食事蛋白質摂取量は，窒素バランスの理論を応用して，排泄される尿素窒素量から次の式により算出できる．食事蛋白質摂取量(g/day)＝［尿素窒素排泄率(g/day)＋0.031×体重(kg)］×6.25．腎機能正常者では尿中クレアチニン排泄量は筋肉量と相関し，尿中3-メチルヒスチジン排泄量の増加は筋肉蛋白の分解の亢進を表すが，腎疾患で糸球体濾過量低下を認める場合には，筋肉量とは無関係にこれらの尿中排泄量排泄が低下するので，栄養状態の指標とすることはできない．〔臨床検査　50：521-523，2006〕
