腸球菌(臨床編)

はじめに

　腸球菌はヒトや動物の腸管内に常在する菌であり，病原性は必ずしも高くないので感染症として治療する機会は多くない．しかし，最近では病院感染症の主たる原因となっており，易感染症患者の菌血症やカテーテル留置患者の尿路感染症の原因菌として重要である^1～8)．グリコペプチド系抗菌薬であるバンコマイシンは，腸球菌に対して優れた抗菌活性を有している．しかし，欧米では1980年代後半にバンコマイシン耐性腸球菌(VRE)が院内感染の原因菌として分離され，その後急速に拡大して院内感染対策上の極めて大きな問題となっている^1～8)．日本では1996年に病院で初めてVREが確認されて以降，VRE感染症および保菌者の報告数は必ずしも多くはないが，すでに全国各地に拡散していると考えられる．VREの院内感染やアウトブレイクも報告されており，院内感染防止対策の強化が求められている^1,2)．日本でのVRE感染症は，2003年11月5日に改正され施行された感染症法において，五類感染症であり全数把握の対象疾患として診断したすべての医師に報告が義務づけられている．つまり，細菌検査室で確実に検出されなければならない耐性菌であり，VREの院内感染を防止するためには，細菌検査に携わる臨床検査技師を始めとする医療従事者の果たすべき役割は大きい．腸球菌は抗菌薬に自然耐性を示すとともに多くの薬剤耐性遺伝子を獲得していることから，細菌検査および腸球菌感染症の治療において留意すべき事項が多い．本稿では，まず治療上問題となる多剤耐性腸球菌について概説し，次に腸球菌感染症の治療法に関してVRE感染症の治療法を中心に記述する．
