大腸菌(臨床編)

　はじめに

　大腸菌は，人の腸内常在菌であり，市中感染としては主として尿路感染の起炎菌となり，院内感染としては尿路感染の他，肝胆道感染や下部消化管の創部感染，あるいは肺炎の起炎菌としても分離される．一般的に抗菌薬には感受性であり，第一世代セフェムやキノロン系抗菌薬に良好な感受性を示す．しかし，新たなβ-ラクタマーゼの獲得や遺伝子変異によりこれらの薬剤に耐性を示す株が増加傾向にある．本稿では，このような薬剤耐性の現状および治療と感染予防策について述べる．
