T波交互脈

文献購入ページに移動 T波交互脈(T-wave alternans;TWA)とは,形の異なるT波が1拍ごとに交互にみられる現象である.スペクトル解析によって検出されるマイクロボルトTWAが心臓突然死の予知指標として活用されている.最近では,Holter心電図をタイムドメイン解析することでTWAを検出する解析プログラムも発売されている.TWAの予知指標としての特徴は,不整脈事故に対して陰性的中率が極めて高いことであり,心筋梗塞/虚血性心筋症や拡張型心筋症/心不全において多くのエビデンスが出されている.
