QT延長症候群(先天性，後天性)

　QT延長症候群(LQTS)は心電図上，著しいQT時間の延長と特異な多形性心室頻拍(TdP)を呈する遺伝性疾患である．先天性LQTSでは，すでに九つの遺伝子型が報告され，特に頻度の高いLQT1，LQT2，LQT3では遺伝子型に基づいた患者の生活指導や遺伝子型特異的治療が実践されつつある．また，薬剤，電解質異常などを原因とする後天性LQTSの一部でも先天性LQTSの原因遺伝子の異常が同定されている．本稿ではその分子生物学的背景と臨床病態および治療について，先天性QT延長症候群を中心に概説する．
