血中に分泌された性ステロイドホルモンは標的臓器に選択的に取り込まれた後，細胞質に存在するステロイドホルモン受容体と結合し，複合体を形成する．次いで核内に進入し，クロマチンと結合することによってmRNAを合成し，さらにリボソームによって新しい蛋白合成へ進展し，種々の機能を発現する．性ステロイドホルモンは生物学的にエストロゲン・プロジェストーゲン・アンドロゲンの3つに分類される．プロゲストーゲンとは黄体ホルモン(luteinizing hormone；LH)作用を示す性ステロイドホルモンの総称であり，プロゲステロンは天然に存在するプロゲストーゲンである．
生殖年齢の女性において血中プロゲステロン濃度は黄体の形成や退行に伴いダイナミックに変化する．プロゲステロンは妊娠の成立・維持に不可欠であり，その産生は正確に調節される必要がある．
本稿では，プロゲステロンの構造・生合成，代謝，産生・分泌，異常値を示す病態につき解説し，さらにプロゲステロン受容体と疾患に関する最新の知見に関しても簡単に解説することとする．
