6) ナトリウム利尿ペプチド

はじめに

　生物は進化の過程で体内に塩分をため込む能力を獲得してきた．そして腎臓の尿細管系が生まれ，次第に複雑化し，またレニン-アンジオテンシン-アルドステロン系(renin-angiotensin-aldosterone system；RAA系)を進化させてきた．それと同時に，体内に過剰となった塩分と水分を速やかに体外に排泄させる機構も発展させてきた．その代表がナトリウム利尿ペプチド系(natriuretic peptide system)である．

　現在RAA系とともに，腎に作用して体液の量と組成および血圧の調節を司る主たる液性因子としてのナトリウム利尿ペプチド系の重要性は論を待たない．そして3種類のナトリウム利尿ペプチドファミリーのうち，心房性ナトリウム利尿ペプチド(atrial natriuretic peptide；ANP)は心房から，脳性(B型)ナトリウム利尿ペプチド〔brain(B-type)natriuretic peptide；BNP〕は心室から主に分泌される心臓ホルモンとして，また，C型ナトリウム利尿ペプチド(C-type natriuretic peptide；CNP)は血管内皮や骨，マクロファージや腎尿細管における局所ホルモンとして，それぞれ特異的受容体に働いて作用を発揮することが示されている(図1)^1,2)．さらに，ナトリウム利尿ペプチドファミリーは心不全や高血圧に際して産生・分泌が亢進し，代償的・臓器保護的に作用することが示されてきた^1,2)．特に血中BNP濃度が心機能障害の程度と極めてよく相関し，心不全の診断・予後予測・治療効果判定に有用であることが明らかにされてきた³⁾．

　本稿では，ナトリウム利尿ペプチドの測定および病態における意義について，主に臨床的視点から最近の知見を含め紹介する．
