 アルツハイマー病(AD)と同じアミロイドβ蛋白(Aβ)が脳血管に沈着するAβ型脳アミロイドアンギオパチー(cerebral amyloid angiopathy;CAA)について概説した.孤発性Aβ型CAAは60歳以上の約半数にみられ,AD患者の80~90%にみられる.遺伝性Aβ型CAAはアミロイド前駆体蛋白遺伝子変異を伴い,家族性ADの中にも高度のCAAを伴う例がある.CAAは加齢,ADと密接に関連し,脳内出血等の脳血管障害の原因となる.ADモデルに対するAβ免疫療法により脳出血が誘発されることが報告され,CAAの診断・治療法の開発はますます重要となってきている.
