発症に先立った免疫異常

1．はじめに

　生体においては，外来性の異物が侵入すると免疫反応を介して異物を排除することにより，生体の恒常性を維持している．一方，自己成分は免疫学的寛容と呼ばれる機序により，免疫反応から攻撃を受けないように守られている．しかし，生体における免疫学的寛容が破綻すると，自己成分に対する免疫反応が惹起され，自己免疫疾患が発症する．自己免疫疾患では，T細胞などの免疫担当細胞の異常に加え，血清中に自己成分に対する様々な抗体(自己抗体)が出現する．自己抗体は必ずしも自己免疫疾患の症状や臓器障害を引き起こす原因物質ではない．しかし，血清中の自己抗体の存在は，自己免疫疾患の診断や予後判定などに重要な情報となる．近年，発症前の患者血清中にも自己抗体がすでに存在することから，自己抗体の測定は疾患の発症予測にも有用であるとの報告がなされている^1,2)．本稿では，発症に先立った代表的な免疫異常について解説する．
