はじめに

　深在性真菌症の多くは易感染性患者に日和見感染症として発症する．近年，医療技術の進歩により，先進的医療が多くの施設で施行されるようになり，医療施設においてはこれまで以上に日和見感染症に罹患するリスクを有する患者が多く存在する．

　医療現場で問題となる酵母様真菌は主にカンジダ属であり，本菌は皮膚，口腔，消化管，尿生殖器に常在する．従来，カンジダ症を含めた真菌症が院内感染制御の対象として認識されることはあまりなかった．しかし，近年，Candida parapsilosisのICU，NICU，救急施設における集団感染の事例なども報告されており^1,2)，アスペルギルスも含めて，感染制御の対象として認識する必要性が出てきている．また，カンジダ症に関する臨床的な問題点としては，アゾール系抗真菌薬に低感受性のnon-albicans Candidaの検出頻度が増高くなってきていることも挙げられ，施設内で検出されるカンジダの菌種，薬剤感受性についても，施設ごとにモニターしていくことも重要である．

　従来から感染制御の対象として代表的なメチシリン耐性黄色ブドウ球菌(methicillin-resistant Staphlococcus aureus；MRSA)や多剤耐性緑膿菌(multiple drug-resistant Pseudomonas aeruginosa；MDRP)などの細菌感染症と同様に，カンジダ症においても，医師のみならず，臨床検査技師，薬剤師も含めたコメディカルと協力しあい，感染制御を行っていくことが肝要である．本稿では，これらを踏まえたうえで，特にカンジダ血症について概説する．
