はじめに

　ノロウイルスは，医療関連施設内における急性胃腸炎の集団発生の原因として重要である．このことは，2004年12月に福山市の特別養護老人ホームにおいて入所者7名がノロウイルス感染症により亡くなるという事件があって以来，社会一般の関心を集めるようになった．また，これを機に，厚生労働省が2004年11月以降2005年1月12日までの高齢者特別養護施設におけるノロウイルス感染症の緊急実態調査を行った．その結果，236施設中，疑い例を含め，約5,400例のノロウイルス陽性例があり，12名の死亡例があることが判明した．これはノロウイルス性食中毒の集団発生事例での死亡者が皆無であることとは対照的であり，社会におけるノロウイルス感染症の認識を一変させた．

　ロタウイルスは，乳幼児期の入院治療を必要とする重篤な急性胃腸炎の約半数の原因になっている．最近，ロタウイルス胃腸炎を予防するためのワクチンが過去30年にわたる努力の末に完成し，世界の多くの国で認可されている¹⁾．また，ロタウイルス胃腸炎による小児科入院の20～30%が院内感染に起因していることが知られている．

　本稿では，ロタウイルスとノロウイルスの基本的特徴について簡単に説明し，これらのウイルス感染症の感染制御について，臨床検査のポイントに言及して，解説する．
