抗血小板薬であるチエノピリジン系薬剤は，心筋梗塞や脳梗塞といった動脈硬化性疾患の予防に非常に重要な薬剤であり，CAPRIE1）をはじめとする大規模臨床試験によってその有効性が明らかとなっている．特に虚血性心疾患に対する経皮的冠動脈形成術(percutaneous coronary intervention；PCI)においては，ステント血栓症予防のためアスピリンおよびチエノピリジン2剤併用による抗血小板療法が必須となっている．しかし，抗血小板療法を行っているにもかかわらずステント血栓症を発症する例があり，抗血小板薬に対する不応性の存在が注目されている．
クロピドグレルは，世界中で広く用いられているチエノピリジン系抗血小板薬であるが，その効果には個人差があることが報告されている2）．クロピドグレル不応症の原因の1つとして，薬物代謝酵素であるチトクロームP450(CYP)の遺伝子多型が関与しており，その抗血小板作用や臨床イベントとの関連について概説する．
